Jon J. Breen, APN, DNP, FNP-BC

Jon J. Breen, DNP, FNP-BC, APN, is a family nurse practitioner who provides patient-centered primary care with a focus on prevention, evidence-based treatment, and respectful communication. He supports adults across a wide range of health needs, including chronic disease management, acute concerns, and health maintenance visits. Jon has specialized experience in HIV care, HIV prevention (PrEP), and STI evaluation and treatment, and he is committed to creating an affirming, welcoming care experience for LGBTQ+ and gender-diverse patients. His background also includes emergency department and cardiac step-down nursing, as well as community-based outreach through street medicine, helping patients connect to care and follow-up resources.
